• Your name in the name field
• The name of the person requiring healing in the subject field
• The specific healing request, if desired, in the message field
• When completed click send. 

When your request is received, it is added to our current list and will remain on the list for thirty days. Resubmit your request with an update if required after that time. The list is emailed to our team, all of whom have signed a statement of confidentiality regarding your request.
